DOH UPDATE ON MEXICAN SWINE FLU
Early this morning, the World Health Organization (WHO) raised the pandemic alert from Phase 4 to Phase 5 after seven more countries aside from Mexico and the United States confirmed cases of the novel swine influenza A (H1N1) virus bringing the total to 148 cases and 7 deaths. In the Philippines, there has been no documented case or death due to the virus.

Phase 5 means that there is now “human to human transmission of the virus causing sustained community outbreaks in at least two countries in one WHO region”. In this scenario, it is expected that air travel can further spread illness thus increasing the risk of a full-scale pandemic. All countries are now expected to ready the full implementation of their Pandemic Response Plans
Thus, today, I have convened a Command Conference with all the Directors of the Centers for Health Development and the DOH-retained Hospitals including all regional epidemiologists to discuss the readiness plans and command and control systems for an imminent pandemic.
I am also calling for a meeting with the Chiefs of all private hospitals to coordinate with the DOH on how they can prepare and respond to possible cases of the Mexican Swine Flu which may come to their attention.
We will also collaborate with the business sector and members of the pharmaceutical sector on how they can contribute to the National Pandemic Preparedness and Response Plan. This early, we are tapping the local pharmaceutical industry to help us ensure the availability of Oseltamivir when the need arises.
We maintain that there are no current travel restrictions to Mexico, United States and other affected countries. Travelers are strongly advised however, to reconsider trips unless extremely necessary. It is also prudent for incoming passengers to monitor their health status especially if they came from an affected country.
Note the onset of fever and other flu-like symptoms up to 10 days after arrival and immediately consult a physician. Hospitals and medical personnel should report cases of suspected cases of Mexican swine flu to the DOH Hotline at tel. nos. 711-1001 and 711-1002.
We reiterate that pork is safe to eat and that the Mexican Swine Flu is not transmitted via consumption of pork. As a precaution however, the national government through the Department of Agriculture has already issued a “temporary suspension of all hog and pork imports from afflicted zones’ while placing under heightened alert at all airports and seaports to prevent illegal imports.
We also recommend the following personal protective measures:
Cover your nose and mouth with a tissue when coughing or sneezing. Wash hands regularly with soap and water, especially after you cough or sneeze. Alcohol-based hand cleaners are also effective Avoid close contact with sick people.

Based on assessment of all available information, and following several expert consultations, I have decided to raise the current level of influenza pandemic alert from phase 4 to phase 5.
Influenza pandemics must be taken seriously precisely because of their capacity to spread rapidly to every country in the world.

International Health Regulations (IHR)On the positive side, the world is better prepared for an influenza pandemic than at any time in history.
Preparedness measures undertaken because of the threat from H5N1 avian influenza were an investment, and we are now benefitting from this investment.
For the first time in history, we can track the evolution of a pandemic in real-time.
I thank countries who are making the results of their investigations publicly available. This helps us understand the disease.
I am impressed by the work being done by affected countries as they deal with the current outbreaks.
I also want to thank the governments of the USA and Canada for their support to WHO, and to Mexico.
Let me remind you. New diseases are, by definition, poorly understood. Influenza viruses are notorious for their rapid mutation and unpredictable behaviour.
WHO and health authorities in affected countries will not have all the answers immediately, but we will get them.
WHO will be tracking the pandemic at the epidemiological, clinical, and virological levels.
The results of these ongoing assessments will be issued as public health advice, and made publicly available.
All countries should immediately activate their pandemic preparedness plans. Countries should remain on high alert for unusual outbreaks of influenza-like illness and severe pneumonia.
At this stage, effective and essential measures include heightened surveillance, early detection and treatment of cases, and infection control in all health facilities.
This change to a higher phase of alert is a signal to governments, to ministries of health and other ministries, to the pharmaceutical industry and the business community that certain actions should now be undertaken with increased urgency, and at an accelerated pace.
I have reached out to donor countries, to UNITAID, to the GAVI Alliance, the World Bank and others to mobilize resources.
I have reached out to companies manufacturing antiviral drugs to assess capacity and all options for ramping up production.
I have also reached out to influenza vaccine manufacturers that can contribute to the production of a pandemic vaccine.
The biggest question, right now, is this: how severe will the pandemic be, especially now at the start?
It is possible that the full clinical spectrum of this disease goes from mild illness to severe disease. We need to continue to monitor the evolution of the situation to get the specific information and data we need to answer this question.
From past experience, we also know that influenza may cause mild disease in affluent countries, but more severe disease, with higher mortality, in developing countries.
No matter what the situation is, the international community should treat this as a window of opportunity to ramp up preparedness and response.
Above all, this is an opportunity for global solidarity as we look for responses and solutions that benefit all countries, all of humanity. After all, it really is all of humanity that is under threat during a pandemic.
As I have said, we do not have all the answers right now, but we will get them.

In celebration of Labor Day: More than 20,000 jobseekers register for JOBapalooza '09Wednesday, April 29, 2009More and more new graduates and jobseekers are riding on the wave of opportunity by registering with the country's biggest jobs fair dubbed "Tayo Na! Trabaho Na! JOBapalooza '09" Labor Day approaches, the Department of Labor and Employment (DOLE) today reported.
Labor and Employment Secretary Marianito D. Roque said that based on the latest DOLE preliminary report, more than 20,603 jobseekers have already registered for JOBapalooza '09, which shall be held at the SMX Convention Center, Mall of Asia Complex, Pasay City on May 1, 2009.
More than 1,000 local firms and overseas recruiters would offer more than 250,000 productive job opportunities to new graduates and jobseekers.
Following the ribbon cutting ceremony at 8:45 a.m., the main Labor Day program will commence at 9:00 a.m. at the SMX Hall, followed by the launching of the DOLE Hotline and the reading of Labor Day Manifesto, and the awarding of livelihood and training certificates/vouchers to disadvantaged sectors and workers under various DOLE and government thrusts (WIN AP, DOLE-AMP, Phil-Health, Ye-Ye, PGS, and OWWA).

888-INFO-FDAFDA Authorizes Emergency Use of Influenza Medicines, Diagnostic Test in Response to Swine Flu Outbreak in HumansThe U.S. Food and Drug Administration, in response to requests from the U.S. Centers for Disease Control and Prevention, has issued Emergency Use Authorizations (EUAs) to make available to public health and medical personnel important diagnostic and therapeutic tools to identify and respond to the swine flu virus under certain circumstances. The agency issued these EUAs for the use of certain Relenza and Tamiflu antiviral products, and for the rRT-PCR Swine Flu Panel diagnostic test.The EUA authority allows the FDA, based on the evaluation of available data, to authorize the use of unapproved or uncleared medical products or unapproved or uncleared uses of approved or cleared medical products following a determination and declaration of emergency, provided certain criteria are met. The authorization will end when the declaration of emergency is terminated or the authorization revoked by the agency.Currently, Relenza is approved to treat acute uncomplicated illnesses due to influenza in adults and children 7 years and older who have been symptomatic for less than two days, and for the prevention of influenza in adults and children 5 years and older. Tamiflu is approved for the treatment and prevention of influenza in patients 1 year and older.The EUAs allow for Tamiflu also to be used to treat and prevent influenza in children under 1 year, and to provide alternate dosing recommendations for children older than 1 year. In addition, under the EUAs, both medications may be distributed to large segments of the population without complying with the label requirements otherwise applicable to dispensed drugs, and accompanied by written information pertaining to the emergency use. They may also be distributed by a broader range of health care workers, including some public health officials and volunteers, in accordance with applicable state and local laws and/or public health emergency responses.In authorizing an EUA for the rRT-PCR Swine Flu Panel diagnostic test, the FDA has determined that it may be effective in testing samples from individuals diagnosed with influenza A infections, whose virus subtypes cannot be identified by currently available tests. This EUA allows the CDC to distribute the swine flu test to public health and other qualified laboratories that have the needed equipment and the personnel who are trained to perform and interpret the results.The test amplifies the viral genetic material from a nasal or nasopharyngeal swab. A positive result indicates that the patient is presumptively infected with swine flu virus but not the stage of infection. There was no word today from the Public Health Agency of Canada. On Friday, Canada’s Chief Public Health Officer Dr. David Butler-Jones said there were no cases yet in this country.
With the mounting evidence of efficient person-to-person spread, infectious disease experts suggested Canada is likely to see its first cases soon.
“We now apparently have widespread swine H1N1 throughout the United States which tells us that it is highly infectious, therefore having all the makings of the next pandemic strain,” said Dr. Donald Low, chief microbiologist at Toronto’s Mount Sinai Hospital.
“It is just a matter of time before we recognize it here.”
The new strain of swine influenza is likely already in Ontario, said Dr. Michael Gardam, one of Ontario's top infectious disease specialists, because about 60,000 people return to the province from Mexico every month.
"We have to assume that it is circulating in Ontario," he said. "You just have to look at air travel patterns to realize that what goes on in Mexico has to come to Canada."
With the spectre of SARS still looming over the Toronto area, local infectious disease experts are emphasizing the new strain of swine flu should not be compared to the severe acute respiratory syndrome that swept the city in 2003 and killed 44 people.
The "Spanish" influenza pandemic of 1918–1919, which caused ≈50 million deaths worldwide, remains an ominous warning to public health. Many questions about its origins, its unusual epidemiologic features, and the basis of its pathogenicity remain unanswered. The public health implications of the pandemic therefore remain in doubt even as we now grapple with the feared emergence of a pandemic caused by H5N1 or other virus. However, new information about the 1918 virus is emerging, for example, sequencing of the entire genome from archival autopsy tissues. But, the viral genome alone is unlikely to provide answers to some critical questions. Understanding the 1918 pandemic and its implications for future pandemics requires careful experimentation and in-depth historical analysis.
An estimated one third of the world's population (or ≈500 million persons) were infected and had clinically apparent illnesses during the 1918–1919 influenza pandemic. The disease was exceptionally severe. Case-fatality rates were>2.5%, compared to <0.1%>). Total deaths were estimated at ≈50 million and were arguably as high as 100 million.
